1. Aadhaar Number
2. Bank Account Number with Branch Name and IFSC
3. Student’s own mobile number for all future communication
4. Student’s own email address for all future communication
5. Local address with Police Station and PIN, if student’s address is other than permanent address
6. Blood group
7. Scanned copies of:
1. Date of Birth Proof
2. Marksheet 0f 10+2 level
3. Pass Certificate of 10+2 level
4. Caste cerificate( if applicable)
5. disability certificate (if applicable)
6. signature of the applicant
7. Passport size photo of the applicant.
8. Home phone number.
